5. ONDERNEMING / COMMITMENT

Ek doen hiermee aansoek om toelating soos hierbo aangedui. Ek verklaar dat die inligting deur my verskaf waar en korrek is. Sou ek tot studie toegelaat word, verbind ek my tot en onderneem ek om:

i) my neer te l� by die bepalings van die Reglement van die CGTS;

ii) my studiegelde soos in die aangehegte ooreenkoms vervat, te betaal;

iii) die kontakonderrig soos op die semester kalender by te woon, gedagtig daaraan dat dit my semesterpunt mag be�nvloed of, in uitsonderlike gevalle, vooraf altematiewe re�lings met die Seminarium te tref waar bywoning nie moontlik is nie; en

iv) my te onderwerp aan die opsig en begeleiding van 'n mentor wat deur die CGTS aangestel word.

I hereby apply for studies as indicated above. I declare that the information provided by me is true and correct. Should my application be successful, I commit myself and undertake to:

i) submit to the Rules and Regulations of the CRTS,

ii) pay the studyfees according to the attached agreement;

iii) attend the consultation sessions as indicated on the semester calendar, taken into account that it may influence my semester mark or, in exceptional cases, to make alternative arrangements with the Seminary in advance and

iv) submit to the supervision and guidance ofa mentor, which will be appointed by the CRTS.
